Memory support hertfordshire

We are proud to introduce Memory Support Hertfordshire, a brand-new Community Dementia Service launching on 1st October 2024. This service is funded by Hertfordshire County Council and delivered through a unique partnership between Alzheimer's Society, Age UK Hertfordshire, and Carers in Herts.

 

What they offer

Memory Support Hertfordshire is dedicated to providing tailored support for individuals with memory loss, Mild Cognitive Impairment, or dementia, as well as their families and carers. Here’s how we can help:

  • Information and Advice Gateway - The service offers a single point of contact for all your enquiries related to memory loss and dementia. Whether you need advice, support, or information about local services, they are here to help.

  • Group Activities - They provide a variety of groups offering social and engaging activities for people with dementia, Mild Cognitive Impairment, and their carers. These activities are designed to encourage social interaction and enhance wellbeing.

  • Tailored Individual Support - They understand that every person’s journey with dementia is different. Their team provides personalised support to help people live well with dementia, offering practical advice and emotional support. 

  • Carer Support GroupsCaring for someone with dementia can be challenging. Their support groups provide a space for carers to relax, share experiences, and connect with others who understand their situation.

  • Young Onset Dementia Support - They offer specialised support for individuals under 65 who are living with or supporting someone with Young Onset Dementia or rarer forms of dementia.

  • Admiral NursesWhen challenges arise, the Admiral Nurses provide expert, one-on-one support to family carers. Their knowledge and guidance can help navigate difficult times and find practical solutions.

  • Training and Information Sessions - We offer training for carers, individuals living with dementia, and professionals who support them. Additionally, we provide sessions for organisations and communities interested in becoming more dementia-aware.

  • Dementia Friendly Communities - Through the Dementia Friendly Hertfordshire Accreditation Scheme, they work with local communities and businesses to make Hertfordshire a place where people with dementia can live well.

  • Working Together At Memory Support Hertfordshire, we collaborate closely with local partners in the community, as well as with health and social care providers, to ensure the best possible support for those affected by dementia.
